Intake Form

I consent to Naturotherapy sessions (including Somatic and Bioenergetic Therapies), which may involve emotional, physical, or spiritual release. I understand that this work can bring up stored emotions or trauma. I acknowledge that I am responsible for my decisions and results and agree not to hold Rio liable for any outcomes, directly or indirectly, from these sessions. I voluntarily waive any claims or causes of action arising from my sessions with Rio.

I understand that sessions may involve touch, which will be respectful, consensual, and client-driven. I provide written consent for touch during sessions and recognize my right to withdraw consent at any time. I am encouraged to communicate any discomfort and request adjustments as needed.

I acknowledge that Rio is not a licensed physician, psychologist, or medical professional, and that somatic healing sessions are not a substitute for medical or psychological treatment, diagnosis, or advice.

Our sessions are confidential, except in cases where there is a risk of harm to yourself or others, or if required by law, including when contacted by authorities. In such cases, information may be disclosed to ensure safety or comply with legal obligations.

By signing this form, I confirm that I have read, understood, and voluntarily consent to the above terms. I am aware that I have the right to ask questions or discuss any part of this form with the practitioner before proceeding.
